Hospital at Home (H@H)
New care pathway for vulnerable older adults.
Hospital at Home (H@H) offers hospital care at home to vulnerable older adults who would otherwise be hospitalised for acute care. The goal is to prevent hospital-related complications and (postpone) nursing home admissions. The concept is therefore a form of Right Care in the Right Place (Dutch: “Juiste Zorg op de Juiste Plaats” (JZOJP).
Commissioned by the Foundation for Effective Elderly Care (Dutch: Stichting Effectieve Ouderenzorg), Care IQ conducted an evaluation of the implementation and potential further development of H@H. They also investigated whether the lessons learned from H@H could be used to improve the suitability of other JZOJP programs for vulnerable individuals. Conversely, there are also lessons to be learned from successful JZOJP concepts. For the evaluation, dozens of stakeholders from healthcare, businesses, and Alzheimer Nederland were interviewed, supplemented with desktop research.
